The MDS Nurse is responsible for coordinating and completing the facility's MDS/RAI process, ensuring assessments are accurate, timely, and supported by appropriate clinical documentation.
The MDS Nurse works closely with the DON, ADON, nursing staff, therapy, dietary, social services, activities, physicians, residents, and families to ensure resident needs are accurately assessed and reflected in individualized care plans.
MDS experience is preferred, but experienced skilled nursing nurses with strong assessment, documentation, and care-planning experience are encouraged to apply.
The successful MDS Nurse will ensure assessments are accurate, timely, complete, and consistent with the resident's clinical condition and plan of care.
You will help bring together the nursing and interdisciplinary teams to ensure every resident receives an individualized approach to care while supporting quality outcomes and regulatory compliance.
