LongTerm Healthcare is seeking an experienced MDS Nurse to join our skilled nursing team in Illinois. The ideal candidate is an RN or LPN/LVN with strong MDS/RAI experience who can coordinate assessments, care plans, and documentation for long-term residents.

You will collaborate with the DON, ADON, nursing staff, therapy, and families to ensure accurate, timely MDS submissions and compliance with regulatory requirements while supporting quality outcomes.

Qualifications

  • Current RN or LPN/LVN license in good standing, as applicable.
  • Previous MDS experience preferred.
  • Skilled nursing, long-term care, or post-acute experience strongly preferred.
  • Knowledge of MDS 3.0 and the RAI process.
  • Strong understanding of clinical documentation and care planning.
  • Excellent attention to detail.
  • Strong organizational and time-management skills.
  • Ability to manage multiple assessments and deadlines.
  • Excellent written and verbal communication.
  • Comfortable working with electronic medical records.

Responsibilities

  • Coordinate the MDS assessment process for assigned residents.
  • Complete MDS assessments accurately and within required timeframes.
  • Review resident charts and clinical documentation for accuracy and completeness.
  • Gather information from nursing and all applicable interdisciplinary departments.
  • Coordinate interdisciplinary input for MDS assessments.
  • Identify missing, incomplete, or inconsistent documentation and follow up appropriately.
  • Develop and maintain comprehensive, person-centered care plans.
  • Ensure care plans accurately reflect each resident's current condition, needs, goals, and preferences.
  • Participate in care plan meetings and interdisciplinary team meetings.
  • Monitor residents for significant changes in condition.
  • Communicate changes in resident status to appropriate clinical team members.
  • Collaborate with the DON, ADON, Unit Managers, nurses, and other members of the care team.
  • Review documentation for consistency between the resident's clinical condition, MDS, and plan of care.
  • Assist with quality assurance and performance improvement initiatives.
  • Participate in clinical audits and chart reviews.
  • Support survey readiness and regulatory compliance.
  • Maintain accurate MDS and resident records.
  • Stay current with MDS, RAI, and skilled nursing requirements.
  • Communicate professionally with residents and families.
  • Maintain confidentiality of resident information.
  • Assist with identifying opportunities to improve resident outcomes.
  • Perform other MDS and clinical duties as assigned.

MDS Nurse – Skilled Nursing Facility (SNF)

We are seeking a knowledgeable, organized, and detail-oriented MDS Nurse to join our skilled nursing team. The ideal candidate is an RN or LPN/LVN with strong clinical experience who understands the MDS process, care planning, documentation, and the unique needs of long-term care residents.

Position Summary

The MDS Nurse is responsible for coordinating and completing the facility's MDS/RAI process, ensuring assessments are accurate, timely, and supported by appropriate clinical documentation.

The MDS Nurse works closely with the DON, ADON, nursing staff, therapy, dietary, social services, activities, physicians, residents, and families to ensure resident needs are accurately assessed and reflected in individualized care plans.
