Job summary

LongTerm Healthcare seeks an experienced MDS Coordinator (RN) to lead MDS/RAI processes in a skilled nursing setting. You will assess, document, and coordinate care plans in collaboration with nursing, therapy, dietary, social services, and physicians.

The role emphasizes accurate clinical documentation, regulatory compliance, and timely submissions. Strong organizational skills and attention to detail are required to support high-quality resident care and outcomes.

Qualifications

  • Current RN or LPN/LVN license as required by state regulations.
  • Experience with MDS/RAI processes is preferred.

Responsibilities

  • Coordinate the facility's MDS and RAI process.
  • Complete and coordinate MDS assessments within required timeframes.
  • Review resident charts and clinical documentation for accuracy and completeness.
  • Gather information from nursing, therapy, dietary, social services, activities, and other departments.
  • Coordinate interdisciplinary participation in the assessment process.
  • Identify missing, incomplete, or inconsistent documentation and follow up appropriately.
  • Develop and coordinate comprehensive, person-centered care plans.
  • Ensure care plans accurately reflect each resident's current condition, needs, goals, and preferences.
  • Participate in care plan meetings and interdisciplinary team meetings.
  • Monitor residents for significant changes in condition.
  • Communicate changes and clinical concerns to appropriate members of the care team.
  • Review documentation to ensure clinical information accurately supports resident care.
  • Work closely with the DON, ADON, nursing staff, and interdisciplinary team.
  • Assist with clinical audits and quality assurance activities.
  • Monitor MDS-related documentation for accuracy and consistency.
  • Support QAPI and quality improvement initiatives.
  • Assist with survey preparation and regulatory compliance.
  • Maintain knowledge of current MDS, RAI, and skilled nursing requirements.
  • Assist with identifying opportunities to improve resident outcomes.
  • Maintain accurate and organized MDS records.
  • Communicate professionally with residents and families.
  • Maintain confidentiality of resident information.
  • Complete other clinical and MDS-related duties as assigned.

Position Summary

The MDS Coordinator is responsible for coordinating the facility's MDS/RAI process, ensuring assessments are accurate, complete, and submitted within required timeframes.

The MDS Coordinator works closely with nursing, therapy, dietary, social services, activities, physicians, residents, and families to ensure resident needs are accurately assessed, documented, and reflected in individualized care plans.

This position plays an important role in clinical documentation, quality outcomes, care planning, regulatory compliance, and reimbursement.

Responsibilities

Qualifications

  • Previous MDS experience preferred
  • Skilled nursing, long-term care, or post-acute experience preferred
  • Knowledge of MDS 3.0 and the RAI process
  • Strong understanding of clinical documentation and care planning
  • Excellent attention to detail
  • Strong organizational and time-management skills
  • Ability to manage multiple deadlines
  • Strong written and verbal communication skills
  • Comfortable using electronic medical records and computer systems
  • Ability to work independently and as part of an interdisciplinary team
