Qualifications

• RN licensure required in state of practice.
• Bachelor’s degree in Nursing preferred.
• 2 years of nursing experience; Acute care and 1 year Case Management preferred.

Responsibilities

• Demonstrates critical thinking using nursing process to meet client health care needs.
• Establishes collaborative partnerships to assess health care needs and utilization of resources.
• Educates and coaches clients and families about treatment options and resources to support self-care.
• Facilitates communication and coordination among health care team to minimize fragmentation.
