Responsibilities

• Develop and deliver training sessions on quality improvement workflows.
• Educate care coordination on population health concepts and preventive care.
• Provide hands-on Epic EHR training for documentation and workflows.
• Train staff in patient engagement, empathy, and patient-centered care.
• Coordinate outreach to improve preventive screenings and wellness initiatives.
• Collaborate with Quality Improvement and Population Health teams on clinic initiatives.
• Monitor performance metrics and ensure compliance with standards.
• Support design and implementation of population health initiatives.
