Qualifications

• Registered Nurse or Therapist with a current license
• Bachelor’s degree or equivalent professional experience required
• Four (4) years of home health and/or hospice experience required
• OASIS knowledge preferred or certification within 6 months would be beneficial

Responsibilities

• Lead quality assurance, performance improvement (QAPI) initiatives
• Monitor quality data and regulatory compliance across home health and hospice operations
• Provide education and mentoring on quality standards and documentation requirements
• Prepare monthly, quarterly, and annual quality reports and presentations
• Collaborate with leadership and interdisciplinary teams to achieve organizational goals
