Job summary

Community Hospice & Palliative Care in Jacksonville, FL seeks a Quality Improvement Analyst to support the Comprehensive Quality Management Program and provide analytical backing for QA activities. The role requires an RN license and a minimum of five years in quality management, with Lean/Six Sigma preferred; a Bachelor’s degree in nursing or a related field is required.

Qualifications

• Regulatory compliance knowledge (CMS, State and Federal requirements)
• Experience with clinical indicators and outcome measures
• Strong data analysis and reporting skills
• Excellent communication and writing skills
• Proficient in Word, Excel, PowerPoint, Visio
• Ability to manage multiple process improvement projects

Responsibilities

• Reviews quality indicators and patterns to identify quality concerns.
• Participates in statistical analysis and monitoring of clinical data.
• Analyzes data and prepares reports, graphs, and summaries for decision making.
