Job summary

Southwest Community Health Center Inc seeks a Quality Assistant to support patient care and drive metric-driven outcomes through clinical auditing and data entry in an electronic health record environment.

You will collaborate with the Chief Medical Informatics Officer and cross-functional teams, performing data abstraction, reporting, and outreach to close gaps in care while supporting value-based care initiatives.

Qualifications

• Bachelor’s Degree or higher in a medical care field.
• Minimum one year of experience in a health care setting.
• At least 1 year of experience in a care-related quality role preferred.
• Bi-Lingual Spanish speaking a plus

Responsibilities

• Performs clinical auditing and data abstraction from electronic health records and quality improvement data.
• Performs data entry for reporting key metrics to internal and external stakeholders; ensures data accuracy and timeliness.
• Identifies and closes gaps in care through pre-visit planning and outreach with patients and care teams.
• Tracks lab and imaging data as part of pre-visit planning.
• Utilizes population health tools to outreach to patients with gaps in care.
• Participates in data validation of internal and external quality reports.
• Supports performance improvement, population health and telehealth projects.
• Other duties as assigned.
