Job summary

Desert Valley Medical Group in California is seeking a Quality Coordinator to work with the Quality Manager, Medical Director and Administrator on implementing quality platforms, including CCM, TCM and other quality initiatives such as HEDIS, P4P, MIPS, STAR measures and wellness visits.

You will evaluate data, prepare outreach or encounters, educate providers on missing data, and support member services. Travel between clinic offices is required.

Qualifications

• Two years college education or equivalent work experience.
• 1-2 years of IPA/managed care experience preferred.
• Must be able to travel between clinic offices.
• Excellent presentation, verbal and written communication skills, and ability to collaborate with co-workers, senior leadership, and other management.
• Proficient in word processing, spreadsheets (excel), database and typing.

Responsibilities

• Implement quality platforms (CCM, TCM) and other quality initiatives (HEDIS, P4P, MIPS, STAR measures, annual wellness visits).
• Assess missing patient benchmarks and organize data for outreach or encounters with providers.
• Create referrals and educational material related to quality measures.
• Provide backup to the Member Services Representative.
