Job summary

Desert Valley Medical Group, affiliated with Desert Valley Hospital, seeks a Quality Coordinator to support quality initiatives and patient outcomes. The role collaborates with Quality Manager, Medical Director, and Administrator to implement CCM, TCM, and other programs, including HEDIS, P4P, MIPS, and STAR measures.

The coordinator analyzes gaps in care, prepares outreach data, and communicates with providers and patients. Travel between clinic offices is required.

Qualifications

• Two years of college education or equivalent work experience.
• 1-2 years of IPA/managed care experience preferred.
• Must be able to travel between clinic offices.
• Excellent presentation, verbal and written communication skills, and ability to collaborate with co-workers, senior leadership, and other management.
• Proficient in word processing, spreadsheets (excel), database and typing.

Responsibilities

• Implement quality platforms with QA team and leadership.
• Manage CCM/TCM and other quality initiatives including HEDIS, P4P, MIPS, STAR.
• Identify gaps in care and prepare data for outreach or provider encounter.
• Educate providers on closing data gaps and back up Member Services.
