Endeavor Health in Elmhurst, IL is seeking a Quality & Care Transformation Partner for a part-time role. You will lead system-wide quality and process improvement initiatives using evidence-based methods and collaborate with clinical leaders to drive better patient outcomes.

The ideal candidate has 5–7 years of healthcare quality improvement leadership experience, a Bachelor's in a healthcare field (Master's preferred), and certifications such as CPPS/CPHQ or Lean/Six Sigma.

Qualifications

• Bachelor’s degree in healthcare or 7+ years leading healthcare quality initiatives.
• Master’s degree preferred.
• CPPS/CPHQ, LEAN/Six Sigma certification highly preferred.
• 5-7 years of healthcare experience leading quality improvement with training/certification.

Responsibilities

• Lead and manage quality and process improvement initiatives across the system using an evidence-based performance improvement framework.
• Utilize PI tools to optimize outcomes, break down complex processes, reduce waste and errors.
• Engage with stakeholders to address issues and advance projects on schedule.
• Serve as a consultant on regulatory/accreditation requirements and survey support.
• Stay current with evidence-based guidelines and translate them into improvement projects.
• Develop tools to monitor performance and engage executives in improvement efforts.

Hourly Pay Range

$42.54 - $65.94 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Position Highlights

• Position: Quality & Care Transformation Partner
• Location: Elmhurst, IL
• Part Time: 40 hours per week
• Hours: Monday-Friday, 8:00a-4:30p
• Required Travel: Float to all Endeavor locations

What you will need:

Bachelor’s degree in a healthcare related field or a minimum of 7 years of experience leading healthcare quality improvement initiatives. Master’s degree preferred.

Minimum Education Level

• Bachelor’s Degree or higher preferred in a healthcare related field.

Minimum Certification Requirements

• CPPS/CPHQ, LEAN/Six Sigma Certification and/or similar training highly preferred.

Minimum Work Experience

• A minimum of 5-7 years of healthcare experience leading healthcare quality improvement initiatives with formal training and/or certification in PI or team facilitation.

What You Will Do

Performance and Process Improvement Project Management

• Lead and manage quality and process improvement initiatives across the system using an evidence-based performance improvement framework and applying a standardized change management approach.
• Utilizes PI tools proficiently to optimize outcomes to break down complex processes and workflows to understand areas for improvement, minimize waste, and decrease errors.
• Identifies, communicates and engages with key stakeholders required to address issues; moves projects forward according to defined goals and timelines. Identified action steps required when goals not achieved.
• Acts as a consultant with activities pertaining to various regulatory and/or accreditation requirements, encompassing tasks such as document provision, participation in discussions, and offering survey chaperone support.
• Ability to stay up-to-date on the latest evidence-based guidelines and translate and apply those interventions to improvement projects.
• Creates supportive tools as needed to monitor performance and develop proactive approached to enhance quality, safety, and outcomes. Engage executive leaders in development and integration.

Analyzes Data Pertinent to Service Area to Identify Opportunities

• Analyzes data pertinent to identify opportunities for improvement based on internal goals, external/internal benchmarking, industry standards and payor/external regulatory rating systems.
• Interprets and reports data from multiple sources for comparison, benchmarking and performance assessment. Independently seeks out appropriate and timely benchmark data.
• Monitors data for criteria that is selected to evaluate clinical care and patient safety
• Serves as liaison between stakeholders (e.g., clinical leaders, physicians, payers and Health Information Technology), to assure accurate data elements for reporting are identified to maximize reporting effectiveness and utilization.
