Job description

Overview:

Hybrid Position: QBHS will complete CANS screenings for families who are interested in linking with OhioRISE. At least two evenings to 6pm required. Integrated with Outpatient Psychiatry, Outpatient therapy, and Care Managment to support treatment goals. QBHS will also support the Care Managment Clinician through delegated tasks such as pt/family outreach and follow up, resource finding/verification, SDOH screenings, group support, and transition support.

Valid Ohio driver’s license and proof of auto insurance is required by hospital policy. Must pass a motor vehicle background inspection, insurance eligibility, driving qualifications and training set forth by Nationwide Children’s Hospital and must maintain qualification of insurance guidelines.

Job Description Summary:

Supports licensed staff to facilitate therapy and interventions with children and families and assists with the management of psychiatric and behavioral health concerns. Provides skill building, psychoeducation, support, linkage, and referral interventions that promote resilience, independence, and attention to the stabilization of the youth and family unit.

Job Description:

Essential Functions:

  • Supports licensed providers in administering triage, assessment, and other mental health screenings.
  • Facilitates brief crisis interventions, care coordination, safety planning, or other therapeutic interventions to assigned patients under the direction of a licensed clinician. Provides support to group interventions and co-leads groups with licensed clinicians as needed.
  • Provides face-to-face or phone follow-up and support, and short-term case management for youth and families to assist with monitoring of symptoms, stressors, level of risk, safety planning, and de-escalation.
  • Collaborates and communicates with team members, healthcare providers, and families to implement treatment plans and support care coordination.
  • Assists with identifying and building skills and social supports and accessing relevant resources and services.
  • Completes required charting and clinical documentation in accordance with NCH and applicable program standards.
  • Maintains up-to-date professional training and familiarity with research and literature in area of expertise.

Education Requirement:

Bachelor's Degree in relevant field, required.

Licensure Requirement:

QBHS certification to be obtained upon hire, required.

Certifications:

Completion of NCH endorsed Crisis Management Training upon hire, required.

  • Demonstrated skills in providing behavioral health interventions and case management to children and adolescents and their families.
  • Demonstrated ability to exercise a degree of independence, initiative, judgment, and decision-making to prioritize workload and achieve program objectives.
  • Ability to work both independently and collaboratively as a team member to ensure consistent quality of services that support a high level of patient and provider satisfaction.
  • Demonstrated communication and interpersonal skills required to establish and maintain productive and collaborative team and multi-disciplinary relationships.
  • Demonstrates sensitivity, empathy, objectivity, professionalism, effective engagement and customer service skills, and sound judgement in the course of all work duties.
  • Embraces principles of family centered care in which needs, and perspectives of patients and families are acknowledged and honored.

Experience:

One year of providing mental health services with children and adolescents, preferred.
