At Covenant Community Care, we are a faith-based non-profit, Federally Qualified Health Center serving the communities of Detroit with clinics offering integrated medical, dental, and counseling healthcare services. We have an immediate opening for a Full-time Behavioral Health Counselor.

GENERAL DESCRIPTION:

The Behavioral Health Provider will provide integrated and outpatient counseling services within a primary care setting, emphasizing screening, brief interventions, and follow-up care as appropriate.

SPECIFIC DUTIES:
  1. Work as part of an interdisciplinary team to recognize, treat, and manage mental health, substance use, and psychosocial issues.
  2. Support patients in achieving their health goals, including treatment compliance, sleep, and coping with health issues.
  3. Conduct mental health and substance use screenings; provide referrals as needed, including psychiatry, housing, and basic needs.
  4. Provide psychoeducation to patients and families regarding mental health and substance use.
  5. Develop assessments, treatment plans, and evidence-based therapeutic interventions in individual, family, or group settings.
  6. Document patient progress and diagnostic information accurately.
  7. Communicate patient progress with the care team.
  8. Maintain confidentiality in accordance with HIPAA, 42 CFR part 2, and other regulations.
  9. Cover for other behavioral health providers as needed.
  10. Maintain compliance with licensing requirements, including continuing education.
  11. Uphold the Mission of Covenant: "To show and share the love of God..."
  12. Perform other duties as assigned.

Qualifications:
  • Licensed Master’s Social Worker (LMSW) – Required

PHYSICAL DEMANDS:

Must be able to sit, handle, and communicate effectively, with physical ability to lift up to 50 pounds and perform in a potentially loud environment.
