Job summary

Jewish Child and Family Services in Chicago, IL seeks a qualified IDP professional to support adults with developmental disabilities in a licensed setting. The role emphasizes Person-Centered planning, compliance with regulations, and collaboration across interdisciplinary teams.

You will lead Discovery phases, contribute to Personal Plans, coordinate CST meetings, and ensure accurate documentation, training, and weekend on-call coverage as part of a dedicated care team.

Qualifications

• Eligible to become a QIDP and complete certification within six months
• Bachelor’s Degree in Human Services required
• Minimum two years’ experience working with adults with developmental disabilities
• Valid Illinois driver’s license and reliable vehicle required for travel between JCFS sites

Responsibilities

• Maintain compliance with agency policies, procedures, and applicable laws
• Lead the Person-Centered Process including Discovery Phase and Personal Plan development
• Facilitate Discovery Phase conversations and implement clear, measurable strategies
• Provide in-real-time coaching to staff to align with plans
• Coordinate CST meetings and revise service plans as needed
• Provide weekend on-call coverage monthly and work one weekend day
