Job summary

Community Resources Staten Island is seeking a QA Specialist/Investigator to join a team dedicated to safeguarding program integrity. You will conduct investigations of reportable incidents, prepare comprehensive written reports, and ensure timely uploads to WSIR and related systems.

The role requires at least 3 years of investigative work, NYS Justice Center/IAC certification, and familiarity with IRMA. A valid NYS driver's license is required, with strong communication and organizational

Qualifications

  • Bachelor's degree in a human services field.
  • 3+ years of investigations and supervision experience.
  • Investigation certification from NYS Justice Center & IAC.
  • Working knowledge of OPWDD's IRMA.
  • Strong oral and written communication skills.
  • Ability to plan, prioritize, and organize workflow.
  • Valid NYS driver's license.

Responsibilities

  • Conduct investigations for reportable and serious incidents.
  • Ensure investigation reports are completed and uploaded within 30 days.
  • Manage 149 investigations and supporting documents in WSIR.
  • Submit incident documentation to Incident Review Committee.
  • Maintain incident management database in Excel.
  • Assist with data for annual trend analyses and reporting.
  • Prepare meeting minutes for IRC.
  • Assist with CAP reviews.
  • Available on-call; may require evenings/weekends.

We are seeking a QA Specialist/Investigator to join our team of caring professionals.

ESSENTIAL RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO:
  • Conducts investigations for all reportable and serious reportable incidents as assigned.
  • Ensure that all written investigation reports are completed and prepared for IRC review and upload to IRMA within the required 30-day timeframe.
  • Uploaded written 149 investigation reports and all relevant supporting documents into the NYS Justice Center's WSIR system.
  • Ensures that all incident related documentation including 149 investigation reports meets acceptable criteria for submission to the Incident Review Committee.
  • Assist with management and tracking of information in OPWDD's IRMA database and ensures timely submission to the Justice Center's WISR and AARM portal to ensure timely incident closure.
  • Secures Justice Center target letters and ensures submission of SCR target searches, and submission forms for Investigative Records for Abuse and Neglect Allegations.
  • Maintains records and database of investigations completed and submits information as requested for preparation of the Annual Trend Analyses and other tracking reports.
  • Reviews all agency internal occurrences, via eVero, and ensures appropriate follow-up.
  • Ensure that all required notifications are completed timely and immediate protection is inputted, via IRMA for reportable and serious reportable incidents.
  • Completes investigations for reportable and serious reportable incidents as needed.
  • Ensures that all incident related documentation including 149 investigation reports meets acceptable criteria for submission to the Incident Review Committee.
  • Maintains Community Resources' Incident Management database in Excel.
  • Assists with compiling data for annual incident trend analysis reports as required by OPwDD and the NYS Department of Health.
  • Attends incident review committee (IRC) meetings and prepares comprehensive meeting minutes.
  • Assist with reviews of corrective action plan (CAP) documentation prior to submission to OPWDD.
  • Completes mailing for CRSI annual satisfaction surveys. Available on-call via agency phone on a 24 hour /7 days per week basis; may require evening and weekend work.
  • Performs additional duties as deemed necessary by the Senior Director of Quality Assurance and Compliance
Qualifications
  • Bachelor's Degree in a Human Services Related field.
  • At least 3 years of prior work-related experience conducting investigations and program supervision.
  • Investigation Certification from the New York State Justice Center and the Inter-Agency Council (IAC)
  • Working knowledge of OPwDD's Incident Reporting Management Application (IRMA)
  • Ability to present facts and information effectively in oral and written form.
  • Independent judgment is required to plan, prioritize, and organize diversified workflow.
  • Valid New York State driver's license
