KOPA, Inc in Jacksonville is seeking a Nurse Practitioner to diagnose and treat health conditions in a skilled nursing setting at Avante Jacksonville. You will be the go-to APRN for clinical care, patient education, and care coordination.

You will conduct exams, order tests, manage treatments, supervise NPs, and help implement workflows. Collaboration with physicians and facility leadership is essential, along with travel to other Florida facilities as needed.

Qualifications

• Must hold an active APRN license as a Nurse Practitioner.
• Minimum 1 year supervisory experience in a skilled nursing facility.
• Experience in primary, acute, and chronic care with strong patient communication.

Responsabilités

• Examine patients and review medical records.
• Order and interpret diagnostic tests.
• Diagnose conditions and propose treatments.
• Prescribe medications and manage care plans.
• Maintain accurate patient records.
• Educate patients and families on diseases and prevention.
