Job summary

Responsibilities

• Provides direct nursing care per policies, procedures and protocols
• Implements and monitors patient care plans
• Monitors, records, and communicates patient condition
• Primary coordinator of all disciplines for well-coordinated patient care
• Notes and carries out physician and nursing orders
• Coordinates patient discharge planning with patient/family and healthcare team
• Assesses learning needs and provides education based on age, culture and willingness to learn
• Performs duties per Nurse Practice Act
