Job summary

The LiveWell Homecare Agency, LLC is seeking a Registered Nurse (RN) for a pulmonary progressive care unit to deliver high-quality, patient-centered care. The role emphasizes assessment, planning, and collaboration with an interprofessional team to promote safe, effective treatment and transitions of care.

You will educate patients and families, develop individualized care plans, and participate in quality improvement and safety initiatives.

Qualifications

• Nursing Diploma or Associates degree and RN license.
• Bachelor's degree in Nursing preferred with 2+ years experience.
• Strong assessment, communication, and critical thinking required.
• Ability to work safely and effectively in a fast-paced environment.

Responsibilities

• Provide direct patient care, including assessment and treatment planning.
• Educate patients and families on health management and post-discharge care.
• Participate in quality improvement and safety initiatives.
• Develop and communicate comprehensive care plans.
• Assess patients’ physical, psychosocial, emotional, cultural, and spiritual needs.
