Job summary

LiveWell Homecare Agency, LLC seeks a Registered Nurse to join a specialized tele-unit within a high-performing hospital. You will assess, diagnose, plan, implement, and evaluate patient care for cardiac and pulmonary patients, emphasizing safety and regulatory standards.

Responsibilities include promoting patient and family-centered care, educating on illness management and medications, developing individualized care plans, and communicating across care teams.

Qualifications

• Nursing Diploma or ADN and RN license required.
• Bachelor's degree in Nursing preferred with at least 2 years of experience.
• Safety-sensitive position requiring alertness and safe work practices.

Responsibilities

• Promotes patient and family-centered care, educating on illness management, home care, and medication administration.
• Participates in activities that ensure patient safety, quality improvement, and professional development.
• Develops and documents individualized care plans with clear goals, communicating effectively across care teams.
• Uses critical nursing skills to assess physical, psychosocial, and emotional needs, respecting patient preferences and cultural considerations.
