Job summary

Brookside Care Center in Stockton, CA, is seeking an experienced Respiratory Therapist to lead and oversee our Pulmonary Program. The Pulmonary Program Manager will build relationships with residents, collaborate with nursing and medical providers, and participate in interdisciplinary care planning to improve resident outcomes.

You will monitor clinical outcomes, review pulmonary-related orders, and establish individualized goals in collaboration with medical providers, residents, and families.

Qualifications

• Minimum of two years of respiratory therapy experience in a hospital, skilled nursing facility, long-term care setting, or related healthcare environment.
• Licensed Respiratory Therapist preferred.
• Experience providing respiratory treatments and managing residents with complex respiratory needs.
• Strong communication and interpersonal skills.
• Ability to work independently while also functioning as a collaborative member of the interdisciplinary team.
• Leadership mindset with the ability to take initiative and follow through.

Responsibilities

• Plan, develop, implement, and evaluate the facility's Pulmonary Program.
• Collaborate with nursing, medical staff, residents, families, and the interdisciplinary team.
• Monitor resident progress and outcomes for the Pulmonary Program.
• Review orders and resident participation on a regular basis.
• Develop individualized goals and determine program duration with providers and residents.
• Participate in interdisciplinary care planning and contribute to resident goals.
• Ensure participants meet clinical and diagnostic criteria for the program.
• Monitor program outcomes and provide reports to leadership.
• Collaborate with Infection Prevention and Control on respiratory infections and data collection.
• Develop and maintain respiratory equipment cleaning, maintenance, and storage processes.
