You will provide clinical leadership, coordinate pulmonary-focused care, and collaborate with the interdisciplinary team to support residents with respiratory and pulmonary needs.

Qualifications

• Active New York State RN license required.
• Experience in skilled nursing, rehabilitation, pulmonary, respiratory, or subacute settings preferred.
• Strong clinical assessment and care coordination skills.
• Experience with care planning and interdisciplinary collaboration preferred.
• Strong leadership, organizational, and communication skills.

Responsibilities

• Oversee and coordinate daily operations of the Pulmonary Program.
• Coordinate individualized care plans for residents with pulmonary and respiratory needs.
• Monitor residents with respiratory and pulmonary conditions.
• Collaborate with physicians, respiratory providers, therapy, nursing, and interdisciplinary teams.
• Assist with admissions, assessments, and care coordination for pulmonary residents.
• Provide staff education on pulmonary care and clinical best practices.
• Ensure compliance with regulatory and clinical standards.
• Participate in quality improvement and program development initiatives.
