Job summary

Taconic Rehabilitation & Nursing at Ulster in Highland, NY is seeking an experienced Pulmonary Program Coordinator (RN) to oversee and coordinate the pulmonary program. You will provide clinical leadership and coordinate pulmonary-focused care with an interdisciplinary team to support residents with respiratory needs.

The position is full-time with an annual salary range of 87,360–91,520 USD, reflecting experience and location.

Qualifications

Active New York State Registered Nurse (RN) license
Skilled nursing, rehabilitation, pulmonary, respiratory, or subacute experience preferred
Strong clinical assessment and care coordination skills
Experience with care planning and interdisciplinary collaboration preferred
Strong leadership, organizational, and communication skills

Responsibilities

Oversee and coordinate daily operations of the Pulmonary Program
Coordinate individualized care plans for residents with pulmonary and respiratory needs
Monitor residents with respiratory and pulmonary conditions
Collaborate with physicians, respiratory providers, therapy, nursing, and interdisciplinary teams
Assist with admissions, assessments, and care coordination for pulmonary residents
Provide staff education on pulmonary care and clinical best practices
Ensure compliance with regulatory and clinical standards
Participate in quality improvement and program development initiatives
