Taconic Rehabilitation & Nursing at Beacon is seeking an experienced Pulmonary Program Coordinator (RN) to oversee and coordinate our Pulmonary Program.

In this role, you'll provide clinical leadership, coordinate pulmonary-focused care, and collaborate with the interdisciplinary team to support residents with respiratory and pulmonary needs.

Qualifications

• Active New York State RN license required.
• Experience in skilled nursing, rehabilitation, pulmonary, respiratory, or subacute settings preferred.
• Strong leadership and care coordination skills.

Responsabilités

• Oversee daily operations of the Pulmonary Program.
• Coordinate individualized care plans for residents with pulmonary and respiratory needs.
• Monitor residents with respiratory and pulmonary conditions.
• Collaborate with physicians, respiratory providers, therapy, nursing, and interdisciplinary teams.
• Assist with admissions, assessments, and care coordination for pulmonary residents.
• Provide staff education on pulmonary care and clinical best practices.
• Ensure compliance with regulatory and clinical standards.
• Participate in quality improvement and program development initiatives.

Salary: $87,360- $91,520 annually (depending on experience)
Hourly Equivalent: $42- $44
Shift: Full-Time

In this role, you'll provide clinical leadership, coordinate pulmonary-focused care, and collaborate with the interdisciplinary team to support residents with respiratory and pulmonary needs.

RESPONSIBILITIES:

REQUIREMENTS:

• Active New York State Registered Nurse (RN) license required
• Skilled nursing, rehabilitation, pulmonary, respiratory, or subacute experience preferred
• Strong clinical assessment and care coordination skills
• Experience with care planning and interdisciplinary collaboration preferred
• Strong leadership, organizational, and communication skills
