Taconic Rehabilitation & Nursing at Hopewell in Hopewell Junction, NY is seeking an experienced Pulmonary Program Coordinator (RN) to lead and coordinate our pulmonary program. You will provide clinical leadership, develop care plans for residents with respiratory needs, and work with an interdisciplinary team to improve patient outcomes.

Active NY RN license required; skilled nursing/rehab/pulmonary experience preferred.

Qualifications

• Active New York State RN license required.
• Experience in skilled nursing, rehabilitation, pulmonary, respiratory, or subacute settings preferred.
• Strong leadership, organization, and communication skills.

Responsibilities

• Oversee and coordinate daily operations of the Pulmonary Program.
• Coordinate individualized care plans for residents with pulmonary and respiratory needs.
• Monitor residents with respiratory and pulmonary conditions.
• Collaborate with physicians, respiratory providers, therapy, nursing, and interdisciplinary teams.
• Assist with admissions, assessments, and care coordination for pulmonary residents.
• Provide staff education on pulmonary care and clinical best practices.
• Ensure compliance with regulatory and clinical standards.
• Participate in quality improvement and program development initiatives.

Taconic Rehabilitation & Nursing at Hopewell is seeking an experienced Pulmonary Program Coordinator (RN) to oversee and coordinate our Pulmonary Program. In this role, you'll provide clinical leadership, coordinate pulmonary-focused care, and collaborate with the interdisciplinary team to support residents with respiratory and pulmonary needs.

• Strong leadership, organization, and communication skills
