Job summary

Nebraska Medicine is hiring a Nursing Care Coordinator for the Pulmonary Medicine Clinic in Omaha. You will connect patients, providers, and multidisciplinary teams to create clear, coordinated care plans across the continuum, from diagnostics to rehabilitation and lifestyle management.

The role emphasizes independent clinical judgment, strong communication, and collaboration with a team to improve lung health outcomes. Full-time, Monday–Friday, 8am–5pm, in-person position in Omaha.

Qualifications

• Minimum of three years RN experience or combination with LPN pathway approved by CNO
• BSN required
• Effective verbal and written communication skills
• Ability to prioritize and make independent clinical judgments
• Experience with basic patient care equipment, policy procedures, and manuals
• Ability to work collaboratively with an interdisciplinary team
• BLS certification from an AHA-endorsed program
• Licensed RN in NE or compact state required

Responsibilities

• Coordinate a patient care delivery system for a designated group of patients in ambulatory and transitional care settings
• Provide targeted interventions to avoid hospitalization and ER visits
• Support patients through testing, treatment, rehabilitation, and lifestyle management
