Qualifications

• Active enrollment in a graduate program with transcript requirements.
• Verification statements must be provided for related credentials.

Responsibilities

• Assess nutritional status of women, infants, and children from health records and questionnaires.
• Provide counseling and develop care plans for high-risk individuals.
• Work with families to set goals using Motivational Interviewing.
• Record SOAP documentation in EMR.
• Make referrals and coordinate with healthcare providers and community agencies.
