Job summary

Eagle County Public Health and Environment is seeking a Nurse Home Visiting Nurse Supervisor to lead NFP and Family Connects programs, oversee grants, data systems, and clinical supervision. The role emphasizes health equity, team leadership, and collaboration with partners to reduce disparities in Eagle County.

The position offers schedule stability with a Monday–Friday, 8am–5pm routine and requires Colorado RN licensure, BSN, CPR, and driving.

Qualifications

• RN licensure in Colorado required.
• BSN from an accredited college or university required.
• Masters in Public Health or Nursing preferred.
• Two years of maternal/child health, public health, home visiting or mental/behavioral nursing experience preferred.
• Current CPR certification required.
• Valid Colorado driver’s license required.

Responsibilities

• Oversee grant deliverables, program fidelity, policies, and budgets for NFP and Family Connects.
• Manage data collection systems for NFP and Family Connects and analyze metrics to drive quality improvement.
• Utilize data on social determinants of health to inform policies and address community care needs.
• Recruit, train, supervise, and evaluate professional nursing and paraprofessional staff.
• Provide reflective clinical practice and support team culture; resolve conflicts.
• Coordinate NFP and Family Connects programs per contract guidelines and standards.
• Conduct field joint visits, supervise clinically, assign caseloads, lead case conferences.
• Lead community coalitions and outreach to address health access disparities.
• Maintain HIPAA confidentiality and coordinate public health clinics and immunization efforts.
• Respond to public health emergencies and support ESF-8 as needed.
