Eagle County Public Health and Environment seeks a Nurse Home Visiting Supervisor to lead the Nurse Family Partnership and Family Connects programs. The role emphasizes population-based nursing, program planning, and evaluation to promote health and prevent disease in the community.

The supervisor will mentor teams, oversee data systems, and drive continuous quality improvement while advancing health equity in Eagle County.

Qualifications

• Registered Nurse licensure in Colorado.
• BSN required; MSN preferred for leadership role.

Responsabilités

• Oversee NFP and Family Connects program delivery per contract guidelines and standards.
• Recruit, train, supervise, and evaluate professional nursing staff.
• Coordinate data collection, analysis and quality improvement across programs.
• Lead community coalitions/advisory boards and collaborate to address health access disparities.
• Maintain HIPAA confidentiality and ensure culturally competent care.
