Job summary

The City of Abilene is seeking a Grants & Community Health Coordinator to work with the ATCPHD. You will coordinate multiple grants focused on chronic disease, obesity, physical activity and nutrition, conduct outreach, collect data, and educate external partners.

Reports to DSHS and participates in calls, meetings and conferences. Auxiliary duties include developing educational materials, presenting public health information, and supporting grant-related proposals.

Qualifications

• Prior experience with grants is preferred.
• On the job training will be provided.
• High School Diploma or GED is required.
• Some college coursework is preferred. Focus areas include education, public health, communications, fitness or related fields.
• A valid Texas driver’s license or ability to obtain one within 90 days is required.

Responsibilities

• Conduct community-based outreach, establish relationships and serve as grant liaison to city departments and outside organizations.
• Assist in the development and delivery of evidence-based educational materials and trainings to internal and external stakeholders.
• Prepare, maintain and submit work plans, evaluation plans, and other special reports to health department leadership and DSHS.
• Under the direction of the Data Analyst, assemble, organize and tabulate statistical data and prepare routine reports.
• Conduct and attend meetings in person and virtually; collect and maintain meeting supporting documentation.
• Perform presentations regarding public health issues and assist with community-based assessments.
• Attend monthly conference calls and annual conferences as required by DSHS.
• Participate in the development and submission of grant documents.
