Job summary

Prowers County Public Health and Environment is seeking an experienced Case Manager to deliver professional long-term care management for Elderly, Blind and Disabled populations, children’s waivers, and related Medicaid-funded services from Lamar, CO. The role involves comprehensive care planning, cross-agency coordination, and on-site client visits.

Qualifications

• Bachelor’s degree in a human behavioral science field is required.
• Five years of LTSS experience or equivalent combination with strong emphasis on human services.
• Experience with Medicaid-funded services and ability to navigate care coordination.

Responsibilities

• Perform intake, screenings, and functional eligibility assessments for LTC services and Medicaid waivers.
• Develop and update individualized care plans and coordinate with home health, behavioral health, and community supports.
• Conduct regular home visits and monitor service effectiveness and client safety.
• Maintain electronic records in CCM system and ensure compliance with regulations.
• Participate in 24/7 public health emergency response as needed.
• Report abuse/self-neglect to adult protection and defend agency positions at hearings.
