Job summary

Evolve Therapy Services, LLC is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Muskego, WI. The PRN position supports the Physical Therapist in delivering resident care and implementing treatment plans.

Responsibilities include screenings, gait and mobility training, and education for residents and staff. Licensure in Wisconsin is required, with ongoing professional development expected.

Qualifications

• Associates Degree in Physical Therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education and licensure maintenance.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluation and treatment planning under supervision.
• Implement treatment plans including strength, mobility, balance, gait, and modalities.
• Monitor resident response and adjust interventions with supervision.
