Job summary

Evolve Therapy Services, LLC in Fort Atkinson, WI is seeking a licensed Physical Therapy Assistant (PTA) for an in-house Skilled Nursing Facility on a PRN basis. You will provide PT services under the direct supervision of the Physical Therapist and help deliver high-quality resident care.

Responsibilities include conducting screenings, assisting in evaluations, developing treatment plans, guiding activity programs, documenting in NetHealth EMR, and collaborating with the care team to maximize

Qualifications

• Active Physical Therapy Assistant license in Wisconsin
• Associate's degree from an accredited program
• Ongoing continuing education and professional development to maintain licensure
• Ability to work under supervision and follow treatment plans

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in evaluations and in developing target goals and treatment plans.
• Assist in group and individual treatment focusing on strength, mobility, balance, gait, and range of motion.
• Document patient progress in NetHealth EMR and communicate with the care team.
• Coordinate schedules with the Physical Therapist and other staff.
