Job summary

Evolve Therapy Services, LLC is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Manitowoc, Wisconsin. The PRN position supports the Physical Therapist in delivering quality resident care and participates in screenings, evaluations, and treatment planning.

The role requires an Associate's degree in Physical Therapy, an active PTA license, and ongoing professional development to maintain licensure.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluation and planning of treatment goals and plans with the Physical Therapist.
• Assist in establishing and modifying resident schedules for therapy.
• Train residents and families in use of equipment and therapeutic techniques.
• Document compliant and medically necessary CMS-appropriate records in NetHealth EMR.

Education

Ongoing continuing education to maintain licensure and certification
