Job summary

Brushy Creek Post Acute is seeking a compassionate Physical Therapist Assistant (PTA) to join our rehab team on a PRN basis in Greer, SC. Under a licensed Physical Therapist, you will deliver skilled therapy to improve mobility, strength, and independence for residents.

You will document progress, contribute to discharge planning, and work with nurses and therapists to ensure high-quality care in a modern, welcoming facility with outdoor spaces.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.
• Experience with PCC and Casamba preferred.
• Strong communication, documentation, and organizational skills.

Responsibilities

• Provide PTA services under supervision of a licensed Physical Therapist.
• Implement individualized treatment plans based on orders and goals.
• Monitor progress and communicate updates to the supervising PT.
• Document sessions, progress notes, and outcomes accurately.
• Assist with discharge planning and caregiver education.
• Collaborate with nursing and therapy teams to improve resident outcomes.
• Maintain a safe, clean, and compliant therapy area.
