Job summary

Brushy Creek Post Acute in Greer, SC is seeking a compassionate Physical Therapist Assistant (PTA) to join the rehabilitation team on a PRN basis. Under the direction of a licensed Physical Therapist, you will provide skilled therapy services to help residents improve mobility, strength, function, and independence.

The position offers PRN scheduling with competitive pay. Experience in skilled nursing or rehab settings and familiarity with PCC/Casamba tools are preferred.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.
• Experience with PCC and Casamba preferred.
• Strong communication, documentation, and organizational skills.
• South Carolina PTA license in good standing is required.

Responsibilities

• Provide physical therapy treatments as directed by the supervising Physical Therapist.
• Implement individualized treatment plans based on physician orders and therapy goals.
• Monitor resident progress and communicate updates to the Physical Therapist.
• Document treatment sessions, progress notes, and resident outcomes accurately and timely.
• Assist with discharge planning and caregiver education.
• Collaborate with nursing staff and interdisciplinary team members to support positive resident outcomes.
• Participate in care conferences and team meetings as needed.
• Maintain a safe, clean, and organized therapy environment.
• Ensure compliance with all state, federal, and facility regulations.
• Help residents achieve their highest level of function and independence.
