Job summary

Sunnyvale Gardens Post Acute in Sunnyvale, CA is seeking compassionate Physical Therapy Assistants (PTAs) to join our rehabilitation team. You will work under the supervision of a licensed Physical Therapist to deliver targeted interventions that help residents regain mobility and independence.

We offer Full-Time, Part-Time, and PRN roles with hourly pay $46–$51 plus comprehensive benefits for eligible positions, including medical, dental, vision, PTO, retirement, and CE opportunities.

Qualifications

• Current California PTA license in good standing.
• Experience in skilled nursing or post-acute rehab preferred.
• Strong communication and collaboration skills.
• Commitment to resident-centered care.

Responsibilities

• Implement therapy treatment plans as directed by the Physical Therapist.
• Provide therapeutic exercises, gait training, and functional mobility interventions.
• Accurately document treatments in compliance with facility and regulatory standards.
• Communicate resident progress, challenges, and recommendations to the therapy team.
• Participate in interdisciplinary care planning.
• Educate residents, families, and caregivers on therapy programs and outcomes.
