Sunnyvale Gardens Post Acute in Sunnyvale, CA is seeking compassionate Physical Therapy Assistants to join our rehabilitation department. We offer Full-Time, Part-Time, and PRN positions with a competitive hourly range and comprehensive benefits.

You will work under a licensed Physical Therapist to deliver targeted interventions, help residents regain mobility, and support quality of life in a resident-centered post-acute setting.

Qualifications

• Current California PTA license in good standing.
• Strong communication, collaboration, and organizational skills.
• Experience in skilled nursing or post-acute rehab preferred, but not required.

Responsibilities

• Implement therapy treatment plans under supervision of a licensed Physical Therapist.
• Provide therapeutic exercises, gait training, and functional mobility interventions.
• Accurately document treatments in compliance with facility and regulatory standards.
• Communicate resident progress, challenges, and recommendations to the therapy team.
• Participate in interdisciplinary care planning.
• Educate residents, families, and caregivers on therapy programs and outcomes.
