Providence Home Care of Florida is seeking an experienced Field PTA to serve our North Pinellas County territory. We are a veteran-owned, locally-operated Medicare-certified Home Health Agency committed to providing high-quality patient care in the comfort of patients' homes.

Job Summary:

A Licensed Physical Therapy Assistant (PTA) administers physical therapy services in patients' homes, following physician orders and plans of care under the supervision of a Registered Physical Therapist (PT).

Qualifications:

• Graduate from an American Physical Therapy Association-approved two-year college program.
• Current license, registration, or certification in Florida.
• Preferred: Two years of experience.
• Ability to exercise initiative and independent judgment.
• Current CPR certification, TB test, health statement/physical clearance, and eligible Level 2 background check.

Responsibilities:

• Follow agency policies and procedures.
• Provide physician-prescribed physical therapy as per the plan of care established by the PT.
• Improve or minimize residual physical disabilities and help patients reach their optimal, productive level.
• Collaborate with healthcare team in patient care planning.
• Perform skilled procedures ordered by the physician and according to the PT's plan of care.
• Consult with PT regarding treatment changes.
• Instruct patients and families on home programs and daily activities.
• Document medical records, progress notes, and care plans promptly.
• Perform services delegated and supervised by the PT.
• Educate patients and families as needed.
• Perform maintenance therapy within the scope of practice and state laws.
