Continuum Therapy Partners in Minnesota is seeking a Physical Therapist Assistant to join our growing network of care locations. In this PRN role you will work under a supervising Physical Therapist to help deliver individualized treatment plans and support patients in skilled care.

You will educate patients on home exercise programs, document progress, and communicate changes to the supervising PT. Requirements include an associate degree, an active PTA license, and at least some prior

Physical Therapist Assistant Requirements & Responsibilities:

• Associate Degree in Physical Therapist Assistant Program required
• Current, unencumbered state Physical Therapist Assistant License
• 1 year of previous experience as a Physical Therapist Assistant preferred
• Implements individualized treatment plans under the supervision of a Physical Therapist
• Documents patient progress and reports changes in patient status to the supervising PT
• Educates patients on home exercise programs and proper use of assistive devices
