Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist Assistant for a PRN daytime role at Lancaster Health Services in Lancaster, Wisconsin. You will work under the supervision of a licensed Physical Therapist to implement individualized treatment plans and monitor patient progress.

This flexible PRN position offers mentorship, patient‑centered care, and the ability to balance professional goals with personal time while contributing to high‑quality rehabilitation services.

Qualifications

• Requires state PTA license.
• 1 year of PTA experience preferred.
• Implements individualized treatment plans under the supervision of a Physical Therapist.

Physical Therapist Assistant Requirements & Responsibilities

• Associate Degree in Physical Therapist Assistant Program required
• Current, unencumbered state Physical Therapist Assistant License
• 1 year of previous experience as a Physical Therapist Assistant preferred
• Implements individualized treatment plans under the supervision of a Physical Therapist
• Documents patient progress and reports changes in patient status to the supervising PT
