Mind & Mobility is seeking a Licensed Physical Therapist Assistant to provide in-home care for Medicare Part B patients under the supervision of a licensed Physical Therapist. You will implement individualized treatment plans, deliver skilled therapy to improve mobility, strength, and function, and educate patients on exercises and safety strategies.

Qualifications

• Active Physical Therapist Assistant (PTA) license in the state of practice.
• Experience in home health or outpatient therapy preferred.
• Knowledge of Medicare Part B documentation and regulations.

Responsibilities

• Assist in implementing individualized treatment plans developed by the supervising Physical Therapist.
• Provide skilled therapy interventions to improve patients' mobility, strength, balance, and function.
• Educate patients and caregivers on exercises, safety techniques, and fall prevention strategies.
• Document patient progress accurately and in compliance with Medicare Part B guidelines.
• Communicate effectively with the supervising Physical Therapist and the interdisciplinary home health/therapy team.
• Monitor patient response to therapy and report progress or concerns to the Physical Therapist.
• Maintain a safe and professional environment during home visits.

Position Summary:
