Job summary

Mind & Mobility Home Care is seeking a Physical Therapist to provide in-home care for Medicare Part B patients in Coconut Creek, Coral Springs, Deerfield and surrounding Broward County areas. You will help patients regain mobility, manage pain, and maintain independence—all from the comfort of their homes.

This PRN, independent contractor role offers flexible scheduling, direct deposit, and support from a collaborative therapy team while complying with Part B documentation.

Qualifications

• Active Physical Therapy license in FL.
• Experience in home health or outpatient therapy preferred.
• Knowledge of Medicare Part B documentation and regulations.

Responsibilities

• Assess patients in their homes and create personalized treatment plans.
• Provide skilled therapy to improve mobility, strength, balance and independence.
• Educate patients and caregivers on exercises, safety and fall prevention.
• Document progress and ensure compliance with Medicare Part B regulations.
• Collaborate with home health/therapy team to coordinate care.
• Adjust treatments based on patient progress and needs.

• Experience in home health or outpatient therapy preferred.
• Knowledge of Medicare Part B documentation and regulations.
• Strong assessment, treatment planning, and communication skills.
• Reliable transportation and valid driver's license.
• Comfortable working independently and managing a flexible schedule.
