Job summary

Mind & Mobility Home Care in Miami is seeking a licensed Physical Therapist to provide in-home Medicare Part B therapy, assessing patients and designing personalized treatment plans to improve mobility and independence. You will deliver skilled therapy in patients’ homes, educate caregivers, and document progress for compliance.

The role is an Independent Contractor (1099) with flexible scheduling, direct deposit, and a supportive team environment, aligning with Florida Care Provider guidelines.

Qualifications

• Active Physical Therapy license in the state of practice.
• Experience in home health or outpatient therapy preferred.
• Knowledge of Medicare Part B documentation and regulations.
• Strong assessment, treatment planning, and communication skills.

Responsibilities

• Assess patients in their homes and create personalized treatment plans.
