HealthPro Heritage in Plaineville, CT is seeking a Physical Therapist Assistant for an inpatient SNF setting with a full-time schedule. You will work within an interdisciplinary team to deliver patient-centered care and monitor progress.

Qualifications

• Associate degree required from an accredited program.
• Valid state licensure as a Physical Therapist Assistant or license eligibility.
• Strong assessment, diagnostic and therapeutic abilities.
• Excellent communication and interpersonal skills; ability to work in a team.

Responsabilités

• Assist in implementing treatment plans designed by Physical Therapists.
• Provide direct patient care including therapeutic exercises, manual therapy, and modality applications.
• Monitor and document patient progress; educate patients and families on protocols.
• Collaborate with Physical Therapists and other staff for coordinated care.
