Tender Touch Rehab Services in Middlebury, CT is hiring a Licensed Physical Therapist Assistant for a full-time role in SNF and long-term care settings. Compensation is $35.00–$42.00 per hour, with full-time schedule and competitive benefits.

You will provide PT services under supervision of a licensed PT, participate in evaluation referrals, maintain documentation, and collaborate with a multidisciplinary team.

Qualifications

• Associate’s degree from an APTA-accredited Physical Therapist Assistant program.
• Current license as Physical Therapist Assistant in the state of practice.
• Strong communication and organizational skills.

Responsibilities

• Provide physical therapy treatments under the supervision of a licensed Physical Therapist, following state practice guidelines.
• Assist in identifying appropriate patients for therapy and coordinate with the PT for evaluation and referral.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meetall regulatory requirements.
• Ensure all required physician orders are obtained and up to date.
