Walnut Creek Nursing and Rehab is seeking a dedicated Physical Therapy Assistant (PTA) to join our team. You will implement treatment plans and assist residents with mobility, transfers, and gait training under the supervision of a Physical Therapist.

Qualifications

• Licensed or certified as a Physical Therapy Assistant in PA
• CPR certification (preferred)
• 1+ year experience in a Skilled Nursing Facility
• Compassion and patience when working with elderly or disabled patients
• Physical ability to assist with patient transfers and provide hands-on therapy

Responsibilities

• Implement physical therapy treatment plans created by the Physical Therapist.
• Assist patients with functional mobility training, including transfers, ambulation, and the use of assistive devices (e.g., walkers, canes).
• Guide patients through strengthening and balance exercises to improve mobility and stability.
• Observe and assess patients during therapy sessions, documenting their responses, progress, and any changes in condition.
• Communicate effectively with patients, families, and caregivers regarding treatment goals and progress.
