Job summary

Evolve Therapy Services, LLC is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Humboldt, TN. You will work under the supervision of a Physical Therapist to deliver patient-centered care and implement treatment plans.

The role requires an Associate's degree and an active PTA license, with ongoing education to maintain licensure. This PRN position focuses on high-quality resident care and CMS-compliant documentation.

Qualifications

• Associate's degree in Physical Therapy required.
• Active Physical Therapy Assistant license in state(s) of practice required.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in development and modification of treatment plans with the Physical Therapist.
• Carry out treatment plans including strengthening, mobility, gait, balance and ROM activities.
• Document compliant and medically necessary CMS-related records in NetHealth EMR.
