Job summary

Evolve Therapy Services, LLC seeks a dedicated Physical Therapy Assistant to support an in-house Skilled Nursing Facility in Newport, TN. You will provide therapy services under the supervision of a Physical Therapist, conduct screenings, and help develop treatment plans to enhance resident mobility and function.

Responsibilities include leading individual and group sessions, monitoring progress, adjusting plans, and ensuring CMS-compliant documentation in NetHealth EMR.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under the supervision of a Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention.
• Assist in evaluation under the direction of the Physical Therapist.
• Develop and modify treatment plans with short- and long-term goals.
• Conduct individual and group treatment sessions as outlined in the treatment plan.
• Monitor resident progress and adjust interventions with PT guidance.
• Assist in resident scheduling and coordination with the care team.
• Ensure timely, compliant documentation in NetHealth EMR.
