Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Southport, CT. This part-time/PRN role supports a licensed Physical Therapist and focuses on resident-centered care, movement training, and functional improvements.

The PTA will conduct screenings, assist in treatment planning, and deliver both individual and group therapies while maintaining CMS-compliant documentation in NetHealth EMR.

Formación

• Associate’s degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state of practice.
• Commitment to ongoing continuing education and licensure maintenance.

Responsabilidades

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings to determine need for intervention/treatment.
• Assist in evaluating and establishing treatment plans with goals.
• Conduct individual and group treatment targeting strength, mobility, balance and gait.
• Monitor resident response and modify plan in consultation with PT.
• Prepare schedules with PT and team; adhere to resident calendar.
• Train residents and staff in equipment use and therapy techniques.
• Document all therapy activities and CMS-required information in NetHealth EMR.
