Job summary

Five Points Nursing is seeking a full-time Physical Therapy Assistant (PTA) to join our in-house rehab team. You will help residents improve mobility, strength, and overall quality of life under the supervision of a Physical Therapist.

Ideal candidates hold a Texas PTA license in good standing, have SNF or long-term care experience preferred, and are familiar with Net Health documentation. We offer competitive pay and benefits with flexible schedules.

Qualifications

• Active and valid Texas PTA license in good standing.
• Strong communication and teamwork abilities.
• Familiarity with Net Health Electronic Documentation System.

Responsibilities

• Administer prescribed PT treatments to relieve pain and restore function.
• Provide skilled PT services under supervision of a PT.
• Relay information to the PT and coordinate care.
• Modify plans of care under PT supervision as needed.
• Measure motor function, strength, and muscle performance.
• Monitor patient response and adjust approaches with the team.
• Educate patients, families, and caregivers on care.
• Ensure timely, accurate documentation per policies.
• Attend meetings designated by the Director of Rehab.
